How to Claim:

Fill out the form below to customize your plaque. Choose to have it presented to you in person at your retirement celebration, or mailed directly to your home.

Eligibility Requirements:

  • 5 Year Member: You must have been a continuous Club Member for at least 5 consecutive years immediately prior to your retirement.
  • Newly Retired: You must submit your request within your first year of retirement.
  • Proof of Retirement: You will need to provide official documentation verifying your retirement date.


Receive In Person:
Club Retirement Plaques are distributed in person at a beautiful ceremony approximately once or twice every quarter. The location and times vary. Your family, friends and former coworkers are invited to attend.

Receive By Mail: If you are unable to attend in person, the Club will arrange to ship your plaque to you. In limited situations the Club may be able to present your Club Retirement Plaque in person at a City-managed or private retirement party. This must be arranged in advance. Contact the Club Employee Recognition Team for more information.

Please allow at least six weeks after approval for your plaque to be ready for distribution or shipping.
